School of Engineering IT Support Request Form

This form is for sending help requests to the School of Engineering IT Support team. Use it if you have a problem with the IT services provided by the School of Engineering. It will first go to the University's central support team. If they can't deal with it they will pass it on to the School of Engineering IT staff.

The fields marked with an asterisk (*) must be filled in, unless they don't apply for your building. The other fields will help us with your problem but are optional.

The computer's details refer to the computer you're contacting us about, not necessarily the one you're using at the moment. If your request isn't about a specific computer, type "N/A" into those fields.

* Computer's w-number in the form wXXXX:
(i.e. the w-number of the machine this request is about)

* Network port's u-number in the form uXXXX:
(If the computer is never connected, type "None")

* Operating system and version:

* Please describe the problem saying what you are trying to do and where it goes wrong. Please quote any error messages you get.

* Subject:

When you have completed the form, press the Submit button.
